Any group or agency outside of the Illinois State University Athletics requesting that a student-athlete(s) participate in a promotional activity must complete this form and submit it for approval 2 weeks prior to the event. If you have questions, please contact Jane Fulton, Associate Athletics Director at 309-438-2750.

NCAA Bylaw 12.5.1.1 allows a member institution or recognized entity thereof (e.g. fraternity, sorority or student government organization), a member conference or a non-institutional charitable, educational or nonprofit agency to use a student-athlete's name, picture or appearance to support its charitable or educational activities. NCAA regulations preclude its members from participating in any fund-raising activities that either directly or indirectly benefit any student who has started ninth grade.
